Most motor vehicle title and tag forms are available from this web site. For a list of our online forms click here. Most forms on this web site may be completed on-line for printing, signing and submission.

Order Forms By Telephone or In-Person

You may order forms from the Department of Revenue, Motor Vehicle Division, by calling (404) 362-6500, pressing "3" and then pressing "2". Leave a message including the form number, form name and the quantity needed as well as your name, address, city, state, zip code and telephone number.

Forms are available during business hours at the Department of Revenue, Motor Vehicle Division's Forms Window located in the lobby at 1200 Tradeport Blvd., Hapeville, GA. 30354, and some are available from your local county tax commissioner's office.
