My Georgia license plate and/or renewal decal is missing or mutilated. How can I obtain a replacement?

First, report the loss or non-receipt of your license plate (tag) and/or decal to law enforcement and obtain a copy of their report. They should not charge for a copy of their report when your tag and/or decal were lost in the mail.

Secondly, apply at the Tax Commissioner's office in the county in Georgia where you reside by submitting the following:

  • A completed MV-7 Application for a Replacement License Plate and/or Renewal Decal - typed or printed in blue or black ink electronically or legibly by-hand in the registered owner(s)' name(s) and signed by all owner(s) individually. If all owner(s) go in person to their county tax commissioner's office with their valid Georgia driver's license or Georgia identification card and the vehicle's description (year model, make and serial number), this application can be completed and printed for signing and processing.
  • A copy of the police report OR
  • Mutilated tag and/or decal, when applicable.
  • Cash, check or money order for $8.00* payable to the Office of the Tax Commissioner.

*Note: The $8.00 fee is not due if the tag and/or renewal decal were lost in the mail prior to the owner(s) receipt, provided the owner(s) apply for a replacement within ninety-days (90) of the tag/decal's issue date and you submit a completed T-200 Form with their MV-7 application.

For special license plates that are lost or mutilated, regular-series license plates will be issued unless you request another special plate on the applicable application and pay the manufacturing fee in addition to the fees and items listed above. Mutilated Prestige License Plates

If your prestige license plate (tag) is mutilated and the letter/numbers are still readable, you may apply for a replacement prestige license plate with the same combination as the original by submitting the following to your County Tax Commissioner's office:

  • The mutilated prestige tag with the letters an/or numbers still readable.
  • A completed MV-9B Request for the Manufacture of a Special Prestige License Plate - typed or printed in blue or black ink electronically or legibly by-hand, except for the signature(s), in the registered owner's name(s) and showing the desired combination. Please read all of the instructions on the front and back of the MV-B application before completing.
  • A completed MV-7 Application for a Replacement License Plate and/or Renewal Decal - printed or typed in blue or black ink in the record owner(s)' name(s) as shown on your original registration and signed by all owner(s) individually.
  • Cash, check or money order for $33 for the total fees due ($8* replacement tag fee and $25 manufacturing fee) payable to the Office of the Tax Commissioner. Please do not remit cash through the mail. Additional fees and/taxes may be due if you are applying for a replacement tag/decal during your registration period.
