My License Plate Registration Certificate, a/k/a Tag Receipt, is Missing or Damaged. How can I obtain a replacement?

To obtain a replacement tag registration certificate, a/k/a tag receipt, submit the following to the Tax Commissioner's office in the county in Georgia where you reside:

  • A completed MV-5 Application for a Replacement Registration Certificate - Except for the signature, this form must be typed, electronically completed and printed or printed legibly by-hand in blue or black ink in the record registered owner(s)' full legal name(s) and signed. If jointly registered, at least one of the registered owners must sign.
    • If all registered owners go in person to their County Tax Agent's office with a description of their vehicle (Year model, make of vehicle, and vehicle identification number,VIN) and his or her valid Georgia driver's license or state-issued identification card, this application can be completed and printed for your signature and processing.
    • Licensed Georgia dealers may apply for the replacement tag registration certificate for their customer. In this case, the dealer should sign the MV-5 application in the space provided for the dealer's signature and show their permanent twelve-digit (12) Motor Vehicle Division assigned customer number.
    • The current vehicle owner(s) may apply for a replacement registration certificate. The current owner(s) should sign the MV-5 application in the spaces provided for the owner(s)' signature(s) and enter the word "current" beside his or her signature. A copy of the document transferring ownership (i.e. Bill of sale, title, etc.) from the registered owner(s) to the current owner(s) must be presented.
  • $1 processing fee. Pay with cash, check or money order payable to the Office of the Tax Commissioner. Please do not submit cash through the mail.

Note: If your license plate (tag) and/or renewal decal is also missing, it will not be necessary for you to apply for a replacement registration certificate since a replacement registration certificate will be issued when the replacement tag/decal is issued.Click here to view what is needed to obtain a replacement tag/decal.

Lost, Stolen or Mutilated IRP Registration Certificates (Cab Card)

If your Georgia IRP cab card is missing or damaged, submit a completed and signed Form MV-142 (Application for a Replacement IRP Cab Card) together with the fee of $1.00. Pay this fee with cash, check or money order payable to the Department of Revenue. Please do not remit cash through the mail.
