How can I obtain a special prestige license plate with my name and/or other letters or numbers?
Before requesting a special prestige license plate (tag), please read all of the instructions. Click here to view this application and/or to electronically complete and print it for signing and submission. Personal prestige tags are not manufactured as souvenirs. To claim the tag, you must register a vehicle with the personal prestige tag. Where to Apply:You may apply for a special prestige tag by submitting the following to your County Tax Commissioner's office. Click here for more information regarding where to apply. What is Needed to Apply• A completed MV-9B Request for the Manufacturer of a Special Prestige License Plate application for each tag being requested. Except for the signature, this application must be typed, legibly hand-printed or electronically completed and printed. Complete the application in the vehicle owner(s)’ full legal name(s). A natural person's full legal name is his or her complete name as it appears on his or her valid § Click here to electronically complete and print the MV-9B application for signing and submission. If you type the application or electronically complete and print it, please use an easy-to-interpret font such as Arial, Times New Roman or Courier. § Enter the full legal name(s), address and county of residence of the motor vehicle owner(s) who are applying for the tag. § Enter the telephone number including the area code or Internet e-mail address, if you would like to be contacted (during business hours) about any problem with your application. § We review each personal prestige tag application. We use several tools to minimize the chances of manufacturing tags that are obscene, profane or defamatory in nature. If a personal prestige plate combination is approved and later determined to be obscene, profane or defamatory in nature, DOR reserves the right to recall the tag. If a tag is recalled, we will allow you to request another personal prestige tag combination. § If the letters and numbers on a requested tag combination are less than seven (7) for a private passenger vehicle and less than six (6) for a motorcycle, be sure and leave the boxes blank where you want the spaces to appear on the tag. Every space (blank) is counted as a letter or number. § The following letter and number combinations will not be approved for a personal prestige tag combination: Three (3) letters and four (4) numbers; two (2) letters with one(1), two(2), three (3), four (4), or five (5) numbers. § The following letter combinations will not be approved for a personal prestige tag: GBI, FBI, GSP or GOV. § Personal prestige tag combinations that are registered trademarks will not be approved unless you have written permission from the person or business that has registered the trademark or owns the rights to the trademark. § If you are completing this application by-hand, please make sure that the letters and/or numbers are legible, cannot be misinterpreted and they are printed as follows: A, B, C, D, E, F, G, H, I, J, K, L, M, N. O, P, Q, R, S, T, U, V, W, X, Y, Z 0, 1, 2, 3, 4, 5, 6, 7, 8, 9 § Personal prestige tag combinations for private passenger vehicles weighing less than 14,000 pounds cannot exceed seven (7) letters and/or numbers including spaces. Personal prestige tag combinations for motorcycles cannot exceed six (6) letters and/or numbers including spaces. § No punctuation or symbols are allowed. Only letters, numbers and spaces are allowed. Symbols include but are not limited to the following: Period (.), dash (-), comma (,), exclamation point (!), colon (:), plus sign (+), ampersand (&), etc. The tag manufacturing die for the letter "O" and the number zero are the same. § Please be sure to check the box indicating the type of tag you are requesting: Passenger vehicle (weighing 14,000 pounds or less) or Motorcycle. Please do not check both boxes! • A $25.00 manufacturing fee must be submitted for each plate being requested. § Pay all fees due with cash, check or money order payable to the Office of the Tax Commissioner. § Please do not submit cash through the mail! If you are applying for a prestige tag during your registration period, you must also comply with all of the requirements of registration that includes paying any applicable registration fees and ad valorem taxes due, providing proof of insurance and proof that your vehicle has passed an emission's inspection, if required for your vehicle in your county. Please do not lose track of it! Important• If you pre-paid the special prestige tag manufacturing fee, please include the receipt or a copy with the registration renewal application, registration fees and/or applicable ad valorem taxes at the time you register or renew your vehicle’s registration. • Registration renewal fees are: ü $20.00 annual tag fee, ü $25.00 annual special tag fee, ü Ad valorem tax, ü Any penalties due for late renewal, and, ü $1.00 if the plate is to be mailed to you. • If you did not pre-pay the manufacturing fee, registration fees are: ü $25.00 manufacturing fee, ü $20.00 annual tag fee, ü $25.00 annual special tag fee, ü Ad valorem tax, ü Any penalties for late renewal, and, ü $1.00 if your plate/decal is to be mailed to you.
