My IRP license plate (tag) is lost, stolen or mutilated.
How can I obtain a replacement?
First, report the loss or non-receipt of your IRP
license plate to law enforcement and obtain a copy of their report.
A copy of their report should be free when the license plate was
lost in the mail.
Secondly, apply for a replacement license plate
by-mail or in-person* at this Department's IRP Unit of the Motor
Vehicle Division by submitting the following together:
- When applying in-person, you must show your valid Georgia
driver's license or Georgia identification card.
- When the license plate has been lost or stolen, submit a
copy of the police report or a completed
T-158 form
when a police report cannot be obtained.
- Mutilated license plate, when applicable.
- A completed and signed MV-141
Application for a Replacement of a Lost, Stolen or Mutilated
IRP License Plate - Except for the signature, type, complete
electronically and print or print legibly by-hand in the registered
owner's legal name. The registered owner must sign this completed
application.
- Replacement tag fee of $8.00* fee. Pay with cash, check,
or money order payable to the Department of Revenue
or submit a completed Form
T-200* when the license plate was lost in the mail. Please
do not remit cash through the mail!

*Note: The $8.00 fee is not
required provided the license plate was lost in the mail and you
apply for a replacement license plate within ninety (90) days of
the plate's issue date and you submit a properly completed and signed
T-200 Form
with your application.
