| What is Needed? |
In-person requests: Valid driver's license, government-issued identification card, a signed written request, or a completed MV-20 (Request for Motor Vehicle Data) form.
Written requests: Completed MV-20 (Request for Motor Vehicle Data) form; proof of interest in vehicle (if required). Please include a self-addressed stamped envelope.

Release of
Insurance Information |
- The name of a motor vehicle's insurer and the insurance status in the state database will only be released upon request from an authorized party along with payment for the required research fees.
- The vehicle's owner, a law enforcement officer, or any other person authorized to receive the insurer's name and the insurance status under the powers of a subpoena approved by the Legal Services Section of the Department of Revenue.
